After solving the Chapel key puzzle you're immediately put in front of another puzzle, this time figuring out the register code. Here it is.
The code is 0414. Once unlocked you'll get a coin and a healing vial. Use the coin in the vending machine inside the Chapel and solve the Simon Says puzzle. Once done you'll get a bag of blood.
